Order Form and Instructions

To place orders for our merchandise, please submit the WSPMF Order Form (below) with your money order to the address on the left. Money orders should be made payable to the Washington State Patrol Memorial Foundation. For call-in orders, dial 1-888-576-0670 and have your Visa or MasterCard ready with your list of items.

Orders will have a $6.00 shipping and handling fee, $10.00 for out of state orders. For orders under $10.00, add only $2.50. Sales tax has been included in the price of all of the Foundation merchandise.

As with any merchandising program, delays may occur. We will make every effort to keep delays to a minimum and will do our best to provide quality service and products.

Our merchandise may change periodically as we identify what items our customers are most interested in, and as new items become available. You can also send your suggestions for new items to our E-mail address at wspmf@thurston.com.
